20 year female with fever and generalized body pains

C/O fever since 2 months 

Generalized body pains associated with chills and rigors

Morning stiffness since 2 months 

Blurring of vision and severe headache since 2 months

HOPI: 

Pt was apparently asymptomatic 2 months back , then she developed fever, generalized body pains and severe headache 

History of past illness:

Not significant

N/K/C/O of DM, HTN, Epilepsy,  TB, Asthma

GENERAL EXAMINATION: no pallor, icterus , cyanosis, clubbing , lymphadenopathy , edema 

Temp : 97.4 f

Pr :92 bpm

Bp:90/50 mmhg

Spo2: 99%

SYSTEMIC EXAMINATION :

RS : BAE+  , normal vesicular breath sounds , 

 

P/A : soft , non tender , bowel sounds present

 

CVS : S1 S2 + , no murmurs 

CNS : NAD

DIAGNOSIS: 

? Pyrexia of unknown origin 

? Spondyloarthropathy 

Treatment:

1) Tab.Dolo 650 MG/PO/QID

2) INJ.NEOMAL 1GM/IV/SOS

3)TAB.NAPROXEN 250MG/PO/BD

4)INJ.OPTINEURON 1AMP IN 100ML NS/IV/OD

5)BP/PR/TEMP 4TH HOURLY 

6)STRICT I/O CHARTING
